Are you or a loved one living with diabetes? We can help. Our Rhode Island Diabetes Center offers support services and education programs to help you live better.
Diabetes Education
Our certified diabetes educators are partners in your care. They will give you the latest information and techniques for managing your diet, monitoring your blood-glucose, administering medications, as well as tips for exercise, foot care and other key aspects of diabetes management and treatment. Private diabetes management consultations are available with a physician referral. Prior to attending our diabetes management classes, you will need to schedule a one-on-one session with our South County Hospital Diabetes Center Coordinator, April Silvestro. April (right) is a National Board Certified Diabetes Nurse Educator with 27 years' experience as a nurse and 23 years' experience as a diabetes educator. April provides both individual and group sessions on diabetes, and is a co-leader of the Diabetes Management and Support Groups. April can be reached at 401 788-1696.
Diabetes Management & Support
South County Hospital, in Wakefield, RI, offers on-going support groups for adults and children with diabetes, as well as their families. These groups are led by members of the Hospital's diabetes education team, including registered dietitians, nurses and clinical exercise physiologists. Consider joining a support group.

If your diabetes management includes wound care, The Wound Care Center at South County Hospital offers state-of-the-art therapies and the most advanced treatments available for non-healing wounds caused by diabetes. Our healing outcomes surpass the national benchmarks for excellence.
