At the South County Hospital Diabetes Center, you will find everything you need to manage your Type 1 or Type 2 diabetes and lead an active, healthy life. Our holistic, personalized approach is tailored to help southern Rhode Islanders with diabetes meet individual goals. And our dedicated staff of expert diabetes educators offer a wealth of knowledge and experience to help you manage your condition, prevent complications, and make healthy lifestyle choices. 

Getting Started

Ask your physician for a referral to the South County Hospital Diabetes Center and you’re on your way to healthier living. On your first visit to the Diabetes Center, we will meet with you individually to assess your diabetes
and unique needs. We will give you the tips, tools, and techniques to optimize your health and empower you to make and sustain healthy lifestyle choices. Our goal is to reduce your risk of diabetes complications.
South County Endocrinology provides specialized treatment for southern Rhode Islanders with diabetes and a host of other illnesses caused by hormonal and metabolic imbalances.  Endocrine disorders often are complex and involve many parts and processes of the body, particularly the delicate balance of hormones.

Diabetes Education

To help you manage your diabetes, we give you the option of individual education visits, group classes, or a combination of both. Our programs will help you and your family learn important self-management skills, including:

  • Understanding diabetes
  • When and what to eat
  • Taming the ups and downs of blood sugar levels
  • Diabetes medications
  • Activities and exercises to lose weight and stay healthy
  • Stress management
  • How to cope
  • Taking control and preventing complications
  • How to make healthy lifestyle changes
  • Gestational diabetes management
  • And much more!

       April Silvestro is a National Board Certified Diabetes
         Nurse Educator with nearly 30 years' experience as a
         nurse and more than 20 years' experience as a diabetes
         educator.

Prior to attending our diabetes management classes, you will need to schedule a one-on-one session with our South County Hospital Diabetes Center Coordinator, April Silvestro.  April is a National Board Certified Diabetes Nurse Educator with nearly 30 years' experience as a nurse and more than 20 years' experience as a diabetes educator. April provides both individual and group sessions on diabetes, and is a co-leader of the Diabetes Management and Support Groups.

Diabetes Management & Support

In addition, the South County Hospital Diabetes Center offers free support groups for adults and children with diabetes, as well as their families. Members of the Hospital’s diabetes education team, including registered dietitians, nurses, and clinical exercise physiologists, lead our support groups. Join us for ongoing education. Come give and get support! Consider joining a support group.

Wound Care

If your diabetes management includes wound care, The Wound Care Center at South County Hospital offers state-of-the-art therapies and the most advanced treatments available for non-healing wounds caused by diabetes. Our healing outcomes surpass the national benchmarks for excellence. In one year, 98.7% of more than 500 wounds seen were healed within about 21 days after the patients' first visit to the Wound Care Center.
